Pharmacology
Esomeprazole is a proton pump inhibitor medication. It reduces stomach acid production by inhibiting a specific enzyme called H+/K+-ATPase in the gastric parietal cells of the stomach. It’s important to note that esomeprazole is the S-isomer of omeprazole, which is a single optical isomer. It controls stomach acid levels better than racemic proton pump inhibitors.
Dosage and Administration | Sergel 20 mg
Esomeprazole is available in two forms: oral capsule and intravenous injection. The dosage details for oral and intravenous esomeprazole are below:
Oral Dosage:
- Treatment of Erosive Oesophagitis:
- 20 mg or 40 mg, once daily, 4-8 weeks.
- Patients usually recover within 4-8 weeks. If not cured in 4-8 weeks may continue for another 4-8 weeks.
- Maintenance of normal status: 20 mg, once daily (clinical studies did not last more than 6 months).
- To relieve symptoms of gastric reflux esophagitis (GERD):
- 20 mg, once daily, 4 weeks.
- If the symptoms persist after 4 weeks, you may consider another 4 weeks of treatment.
- Eradication of Helicobacter pylori:
- Triple therapy to reduce the risk of duodenal ulcer recurrence—esomiprazole 40 mg, once daily, 10 days; Clarithromycin 500 mg, twice daily, 10 days.
- Zollinger-Ellison syndrome:
- 20-80 mg, once daily. Dosage should be individualised, and treatment continued as medically necessary.
- Acid related dyspepsia:
- 20-40 mg, once daily, 2-4 weeks, depending on response.
- Duodenal ulcer:
- 20 mg, once daily, 2-4 weeks.
- Gastric ulcer:
- 20-40 mg, once daily, 4-8 weeks.
IV injection or infusion:
- Gastric reflux ooesophagitis (GERD) with erosive ooesophagitis:
- Adult patients (≥18 years): 20 mg or 40 mg, once daily.
- Pediatric patients (1 year to 17 years):
The intravenous dose of esomeprazole depends on the child’s weight:
- Body weight <5 kg:
- 10 mg, once daily.
- Body weight 5-10 kg:
- 15 mg, once daily.
- Body weight 10-15 kg:
- 20 mg, once daily.
- Body weight 15-20 kg:
- 25 mg, once daily.

Drug interactions
Two liver enzymes, CYP2C19 and CYP3A4, primarily activate Sergel. Studies have shown that Sergel does not usually interfere with the effectiveness of many other medications. Notably, any Sergel complexation with drugs metabolised by CYP 1A2, 2A6, 2C9, 2D6, 2E1, and 3A4 enzyme interaction (drug interaction) is less likely to occur. Studies have also shown that Sergel has no serious interactions with drugs such as phenytoin, warfarin, quinidine, clarithromycin, or amoxicillin.
Although Sergel is generally compatible with many medications, problems may occur in some cases. Sergel may inhibit the CYP2C19 enzyme’s activity. This enzyme mainly metabolises the drug Sergel. For instance, the CYP2C19 enzyme metabolises the drug diazepam. Combine Sergel (30 mg) with diazepam. This action could potentially slow down the body’s elimination of diazepam, leading to a rise in its blood levels.

Overdose effects
A single oral dose of Sergel at 510 mg/kg (about 103 times the human dose on a body surface area basis), has been lethal to rats. The major signs of acute toxicity are reduced motor activity, changes in respiratory frequency, tremor, ataxia, and intermittent clonic convulsions. There have been no reports of overdose with Sergel. No specific antidote for Sergel is known. Since Sergel is extensively protein bound, it is not expected to be removed by dialysis. In the event of overdosage, treatment should be symptomatic and supportive. As with the management of any overdose, the possibility of multiple drug ingestion should be considered.
