Pharmacology  

Rosuvastatin is a selective, competitive inhibitor of HMG-CoA reductase, the enzyme responsible for the rate-limiting step in cholesterol synthesis. It works by preventing the conversion of 3-hydroxy-3-methyl-glutaryl coenzyme A to mevalonate, a precursor of cholesterol. Rosuvastatin modifies lipid levels through two main mechanisms: first, it increases the number of LDL receptors in the liver, which enhances the uptake and breakdown of LDL (low-density lipoprotein). Second, it inhibits the liver’s production of VLDL (very low-density lipoprotein), reducing both VLDL and LDL particles in the bloodstream.

Dosage and Administration  

Heterozygous Familial Hypercholesterolemia in Pediatric Patients (10-17 years):

  • Usual dose range: 5-20 mg/day.
  • Maximum dose: 20 mg/day (doses above 20 mg not studied in this group).
  • Adjust doses every 4 weeks or more based on therapy goals.

Very High Cholesterol and High Risk of MI or Stroke:

  • Recommended dose: 40 mg once daily.

Homozygous Familial Hypercholesterolemia:

  • Starting dose: 20 mg once daily.
  • Adjust treatment based on preapheresis LDL-C levels.

Dosage for Asian Patients:

  • Initiate with 5 mg once daily.

Use with Cyclosporine:

  • Limit Rosuvastatin dose to 5 mg once daily.

Use with Lopinavir/Ritonavir or Atazanavir/Ritonavir:

  • Limit Rosuvastatin dose to 10 mg once daily.

Concomitant Lipid-Lowering Therapy:

  • Increased risk of muscle effects when combined with Niacin or Fenofibrate.
  • Reduce Rosuvastatin dose in these cases.

Combination with Gemfibrozil:

  • Avoid use due to increased Rosuvastatin exposure.
  • If necessary, limit Rosuvastatin dose to 10 mg once daily.

Severe Renal Impairment (ClCr <30 mL/min/1.73 m²):

  • Start at 5 mg once daily.
  • Do not exceed 10 mg daily.

Drug interactions Rosu 5

Rosu 5 mg (Rosuvastatin) has several important drug interactions to be aware of. When taken with Cyclosporine, the concentration of Rosuvastatin increases significantly, requiring the dose to be limited to 5 mg once daily. Similarly, co-administration with Lopinavir/Ritonavir or Atazanavir/Ritonavir boosts Rosuvastatin levels, so the maximum dose should not exceed 10 mg per day. The combination of Rosuvastatin with Gemfibrozil increases the risk of muscle-related side effects, so it’s advisable to avoid using them together, or if necessary, restrict Rosuvastatin to 10 mg daily. Additionally, taking Rosuvastatin with Niacin or Fenofibrate can elevate the risk of muscle problems, warranting dosage adjustments.

Rosuvastatin’s absorption is also affected by antacids containing aluminum or magnesium hydroxide, so it should be taken at least two hours after consuming such antacids. When used with Warfarin, Rosuvastatin may enhance its effects, increasing the risk of bleeding, and necessitating regular monitoring of INR (International Normalized Ratio). Co-administration with Erythromycin can reduce Rosuvastatin concentration, requiring medical consultation. Finally, the use of Fusidic Acid alongside Rosuvastatin increases the risk of muscle issues, making it essential to avoid this combination or closely monitor the patient. 

Side effects Rosu 5  

Rosu 5 mg (Rosuvastatin) may cause a range of side effects, though not everyone experiences them. Common side effects include muscle pain, weakness, and headache. Some individuals may also experience abdominal pain, nausea, and constipation. In rare cases, Rosuvastatin can cause liver problems, leading to symptoms like dark urine, yellowing of the skin or eyes (jaundice), and upper stomach pain.

Other less frequent side effects include dizziness, fatigue, and difficulty sleeping. Some patients might notice increased blood sugar levels, especially if they are at risk for diabetes.
